Successful PHM in 2026 follows a cyclical, five-step process designed to maximize the impact of every healthcare dollar spent.
Step 1: Population Identification: Defining the target group based on geography, insurance enrollment, or specific clinical needs (e.g., "all patients with Type 2 Diabetes in the tri-state area").
Step 2: Health Assessment & Segmentation: Breaking the large population into smaller, manageable "segments" with similar needs, such as "under-5s with asthma" or "seniors over 80 with multiple chronic conditions."
Step 3: Risk Stratification: Using AI to predict which individuals are at the highest risk for a "negative event," such as a heart attack or an ER visit, within the next 12 months.
Step 4: Tailored Intervention: Delivering specific care to the right people. This could be a "nudge" via a mobile app for low-risk patients or intensive "case management" for high-risk patients.
Step 5: Continuous Evaluation: Measuring whether the intervention actually improved health or reduced costs, then refining the strategy for the next cycle.
